When ordering contact lenses, please note these very important points:

Please ensure that the name of the patient is entered if it is different than the name of the person placing the order.

You will be prompted to enter only the type of lens and number of boxes you would like. You do not need to enter a prescription: this will be taken care of by Ilex Eye Associates to ensure that it is up-to-date and accurate. Please ensure that your contact information (e-mail address and phone number) are entered correctly in the event that we need to confirm any details before finalizing your order.

Unless you have the same prescription in both eyes, your order should be for a minimum of two boxes: one for the right eye, and one for the left.
